Re-entry Services to incarcerated and formerly incarcerated:
Project R.A.R.E. staff has provided workshop and seminars within and outside the correctional facilities for many years.  We have assisted those transitioning from prison to community with services such as:
  • Assist offenders in developing a comprehensive reentry plan for community transition
  • Encourage responsible family relationships
  • Encourage compliance with community supervision
  • Referrals to community agencies for services and housing
  • Assistance with completing forms for Department of Social Services and other agencies
  • Assist with job readiness training (i.e. resume development, employment search)
  • Many other services to formerly incarcerated and their families.

CRISIS MANAGEMENT SERVICES

Services for families in crisis:

Low-income families face enormous stresses. The pressure can severely disrupt family life and lead to homelessness, family disruption and separation.  Project R.A.R.E. Case Manager’s will work with families to identify specific need(s) and agency(s) within their area that will be able to assist.  By assessing the client’s current dilemma, thoroughly, it is our endeavor to eliminate the community residents visiting and calling multiple agencies.  Community residents will be availed a one-stop triage location to help them evaluate how and where to receive temporary assistance.

Continuum of Services:

We endeavor to work along side individuals and families to help them reach self sufficiency using strength based service model.  Helping them with their immediate crisis is only part of the process.  Case Managers will help individual families define and meet goals to:
  • develop life skills
  • connect with community resources
  • improve their housing and employment situations
  • nurture and support their children

Providing temporary Relief, finding permanent Answers,  will enable Restoration &  Education to empower individuals, strengthen families and help build strong  communities.
